Emergency Room to Living Room: Guiding Patients to Care

Description: Louisiana has among the highest emergency department utilization rates in the nation, particularly among its Medicaid patient population. In response, the Louisiana Health Care Quality Forum launched an ED registry through its statewide HIE that has yielded significant improvements in non-emergent ED utilization among the state's Medicaid patients. This presentation will demonstrate the lessons learned from this approach and feature the results of UnitedHealthcare Community Health Plan of Louisiana, one of the state's five Managed Care Organizations.

Description: Remote Patient Monitoring (RPM) has been valued by some as a way to improve the quality and efficiency of care while improving provider and patient satisfaction. While there is general belief that these solutions enable healthcare organizations to better manage patients, does the evidence really support these conclusions? With the potential explosion of wearable devices and monitoring in real time a variety of conditions and purposes, is RPM the panacea some think it is? A national survey and case study of its use at CHRISTUS Health will shed light on these questions.
